Job Description
The successful candidate will practice according to the Canadian Nurses Association Advanced Practice Nursing framework to assess, plan, implement and evaluate needs for children/youth with complex wound, ostomy, G-Tube and/or continence issues. Services are provided in consultation and in collaboration with all members of the health care team including those on the Community Discharge Team. The successful candidate will have a demonstrated commitment to evidence-based practices, family centered/multicultural care and evidence of working well within a multidisciplinary team.
The Advanced Practice Nurse will practice according to the framework for Advanced Nursing Practice as set out by the CNA.
Direct Comprehensive Patient Care:
Provide expert nursing care to patients as it pertains to their wound, ostomy and continence (WOC) needs
Optimizing Health Systems:
Review of WOC related safety events (SRS) and provide expert recommendation for systems improvement
Review WOC related metrics through Epic reporting and analyze for improvements to develop the WOC program
Participate and promotes early identification and assessment of the patient’s needs related to WOC needs.
Using evidence based data, lead quality improvement initiatives for the WOC program
Responsible for the development and management of medical directives for the WOC program
Responsible for the development and management of WOC related policy, procedure and standard work
Education:
Provide parent and patient education to patient undergoing treatment as it pertains to their WOC needs.
Develop and deliver curriculum related to the WOC program to CHEO staff, medical staff, and medical residents
Support staff to develop, implement and evaluate the care plan related to WOC issues.
Ongoing mentorship
Research:
Enhances the scientific base of nursing practice by utilizing and supporting research in nursing and with other disciplines
Evaluates nursing and multidisciplinary research and assesses its impact on patient care and nursing services where appropriate
Interprets, communicates and disseminates research findings to support patient care and recommended policy changes based on research
Leadership:
Participate as CHEO Best Practice Spotlight Organization Champion Lead for Pressure Ulcer Prevention.
Provide leadership to CHEO nursing staff as an expert in WOC care.
Consultation and Collaboration:
Provide consultative service to inpatients requiring expert WOC care.
Collaborate with the surgical and medical teams to offer advanced nursing services to patients requiring expert WOC care
Collaborate with the interdisciplinary team (ie Occupational Therapy) to optimize outcomes for patients requiring WOC services.
Work with the inpatient units and Community Discharge Team to support safe and timely discharge
Participate in Medbuy product review related to wound, ostomy, g-tube and continence products
The APN will work closely with nursing leadership to outline the goals of the WOC program and develop an implementation strategy in order to optimize the program in a period of significant growth. The successful candidate will adapt well to change and can manage not only competing priorities, but changing priorities. The APN may be assigned work not outlined above, but as it relates to the wound, ostomy and g-tube program.
Requirements/Qualifications
QUALIFICATIONS
Police Record Check (PRC) (Essential)
Current registration with the College of Nurses (Essential)
Baccalaureate Degree in Nursing (Essential)
Master’s degree in Nursing or a related field (Essential)
Acute care pediatric nursing experience (Preferred)
and/or case management experience 3-5 years (Preferred)
Bilingualism (English/French) (Preferred)
Completion of an NSWOC education program or willingness to obtain (Essential)
Experience managing GTube, ostomies and complex wounds (Essential)
Demonstrated competency in assessment of pediatric health care needs. (Essential)
Demonstrated problem solving skills (Essential)
Ability to work autonomously (Essential)
Effective communication skills including patient advocacy (Essential)
Effective negotiation skills (Essential)
Ability to function effectively within a multidisciplinary team environment (Essential)
Demonstrated leadership skills (Essential)
