As an Advanced Practice Nurse – Wound, Ostomy and Continence (WOC), you will contribute to excellence in the delivery of care to clients requiring support with wound care, ostomy management and continence care within the community serviced by Home and Community Care Support Services South East. You will be collaborating with inter-professional care teams across the health care continuum and provide direct care for individuals in collaboration with a care team. You will also provide face-to-face and virtual visits to patients to collaborate and conduct comprehensive clinical assessments to develop a care plan and set therapeutic service goals. Whether you work in our office, in the community, or in a health care facility, you will play a lead role in providing connected, accessible, patient-centred care – and be supported by our collaborative team that includes over 8,000 regulated health care and other professionals. What will you do? Provide face-to-face and/or virtual visits to patients to collaborate and conduct comprehensive clinical assessments, and consult with patients and other team members, as required, to develop a care plan and set therapeutic service goals. Provide clinical expertise/advice to primary care physicians and community nurses. Provide input and evidence-informed clinical rationales with respect to reassessment and adjustment to care plans, as necessary, based on patient progress. Collaborate on discharge planning for patients, when therapeutic service goals have been met. Teach and delegate specific aspects of patient care to a person or persons deemed competent and capable to carry them out, and accept responsibility for these delegated tasks according to the standards of the College of Nurses of Ontario and Nurses Specialized in Wound, Ostomy and Continence Canada. Collaborate with the patient/family and inter-professional team to design goals that best match the patient’s circumstances. Requirements/Qualifications What must you have? Current unrestricted registration with the College of Nurses of Ontario as a Registered Nurse. Certificate as a Nurse Specialized in Wound, Ostomy and Continence Canada Program (previously known as Enterostomal Therapy Nursing Program). Master’s level education in Wound Healing and Tissue Repair. A minimum of 3 years’ experience in WOC service delivery. Current CPR-Level C & AED certification. A thorough understanding and working knowledge of the Canadian Standards of Practice for Nurses Specialized in Wound, Ostomy and Continence. Knowledge about leading priority interventions for chronic wound care, and implementing the transferable knowledge to Service Provider Organizations (SPOs) and patients, e.g., compression therapy, DFU offloading, and pressure redistribution practices. Thorough knowledge of wound, ostomy and continence products and indications for use. Solid knowledge of health care related legislation and practices. Knowledge of direct care / case management models used in community health care organizations. Excellent interpersonal, communication, assessment, problem-solving, and decision-making skills. A valid Ontario driver’s licence and access to a reliable vehicle. Proficiency in a Windows environment. We have a mandatory COVID-19 vaccination policy. I have been a Team Leader, Manager of Clinical Practice as well as a facilitator for Wounds Canada. I began my journey specializing in wound care around 2005. To further my education, knowledge, skill and ability, I obtained my IIWCC (International, Intraprofessional Wound Care Course certification in 2010 and then the SWAN designation in 2023. I am passionate about providing the best care possible for those I care for along with teaching, supporting and mentoring fellow nurses. I am married, mother of 3, and Oma of 4. Along with my passion for nursing, I have been a volunteer with a local air cadet squadron for the past 13+ years supporting our youth. In my spare time I enjoy spending time with family, friends, gardening as well as drumming in Pipe and Drum Bands.
