This temporary, full-time position, with multi-site accountabilities, would be a great career move for a Registered Nurse or Registered Nurse in the Extended Class committed to excellence in professional practice, who champions best practice ostomy and continence care.
Reporting to the Manager, Professional Practice, you will lead, model, promote and facilitate a best practice approach to Wound, Ostomy & Continence (WOC) care at Niagara Health. Your initial focus as the Advanced Practice Nurse – WOC will be on the enactment of the pressure injury/wound care aspects of the Niagara Health Wound Care Action Plan, while at the same time developing and initiating an action plan for best practice ostomy and continence care. In addition to being an expert resource to the Wound, Ostomy and Continence Team, you will demonstrate leadership throughout Niagara Health with a focus on providing extraordinary caring through knowledge translation, people/change management expertise, clinical staff development and related policy and procedure development for patients at all sites.
As the APN-WOC, you will collaborate and build strong relationships with the WOC Team, Directors, Physician partners, Patient Care Managers, Nurse Educators and other nursing and hospital leaders, to support and engage nurses and inter-professional team members, plus work with community providers to ensure the continuum of care for patients. You will compile, and assist with reporting, WOC metrics, participate in/conduct research and participate in product selection for WOC products, along with providing advanced wound, ostomy and continence care through the assessment, treatment and management of WOC challenges, primarily to inpatients. Providing leadership for various corporate committees, such as the Wound, Ostomy and Continence Committee, and collaborating with the reporting Manager, and the Director of Nursing Professional Practice and Elder Care, will be other key aspects of your role.
To join our team as an Advanced Practice Nurse – Wound, Ostomy & Continence (WOC), you must be an experienced and qualified RN or RN-EC whose knowledge base and skill set include:
Current registration in good standing with the College of Nurses of Ontario as a Registered Nurse (RN) or Registered Nurse in the Extended Class (RN-EC).
Master’s degree in Nursing, Adult Education or Wound Care.
Completion of the International Inter-Professional Wound Care course
Completion of the Nurses Specialized in Wound, Ostomy and Continence – WOC_EP Program achieving the designation of NSWOC
WOCC ( C) Canadian Nurses Association certification is preferred
Wound, Ostomy, and Continence specialty certification.
Five (5) years’ RN and/or APN experience in Medical and/or Surgical nursing in an acute care setting.
Three (3) to five (5) years’ clinical experience in an acute or complex care setting with a direct focus on wound care.
Superior interpersonal and leadership skills, including critical thinking, responsiveness, conflict resolution, change management, visionary thinking, organizational, effective customer service, decision-making and problem-solving skills.
Demonstrated experience in leading teams and enacting a plan of action.
Excellent communication skills, with the ability to relay complex information effectively to patients and health care professionals.
Demonstrated knowledge translation skills to implement evidence-based practices.
Demonstrated experience with quality improvement strategies and methodologies.
Demonstrated proficiency with program development and implementation, project and change management and evaluation.
Commitment to excellence in all elements of professional practice, including excellence in patient care processes, measuring outcomes, collaboration and professional growth through continuing education and research utilization.
Demonstrated ability to be flexible and self-directed, and work independently and as a member of the Professional Practice team.
Demonstrated ability to manage a large/diversified workload with rapidly changing priorities and challenging deadlines.
Computer skills in Word, Excel, Access, PowerPoint and Outlook, and a variety of audiovisual equipment.
Access to reliable transportation and ability to maintain a valid Ontario driver’s license.

Ability to maintain consistent, regular attendance.
