Throughout SWAN® Week, we’ll celebrate and showcase the many ways SWANs are making a difference within their organizations, care teams, and communities, including:

• Integral Members of the Interdisciplinary Team — collaborating with NSWOCs and other health care professionals to strengthen care planning, continuity, and outcomes.

• Positive Impact on Health Care Outcomes — supporting prevention, early identification, timely intervention, and improved outcomes for patients and residents.

• Contributing to Standards and Policy Development  — bringing frontline knowledge and WOC competencies to the development and implementation of policies, protocols, and standards of care.

• Quality Improvement Initiatives — recognizing gaps in care, identifying opportunities for improvement, championing change, and helping turn quality improvement goals into everyday practice.

• Advanced Assessment & Care — bringing enhanced WOC knowledge and assessment skills directly to patients and residents while recognizing when specialized consultation or escalation is required.
