The CNS-NSWOCC role acts as a leadership and change agent role in the advancement of nursing practice and standards; supporting the implementation of interprofessional best practices in the care of patients with wound, ostomy and continence concerns. The key responsibilities of the CLINICAL NURSE SPECIALIST – NSWOCC include: Applying specialized knowledge and expertise on recent advances in nursing best practices to facilitate the effective management of clinical activities, coordination of in/out-patient care and continuity in patient care management through integrated care strategies; Assess and recommend treatment for patients with complex wounds and ostomies; Provides patient and family with education and nursing care as it relates to the diagnosis and treatment plan. Recommends efficacious, cost-effective supplies and equipment for wound, ostomy, and continence care to be utilized by the various UHN acute care and rehabilitation sites. Coaching and mentorship of nursing staff and inter-professional teams to build capacity in wound, ostomy and continence management; Inform and support implementation of preventative pressure injury strategies as part of UHN’s Caring Safely strategy. Lead the development of wound, ostomy and continence policies, procedures, order sets and medical directives as needed Participate in product research, procurement, evaluation processes and committees. Other responsibilities include: Supports quality health care through teaching, mentoring, quality monitoring and program development.
