The theme of SWAN™ Week 2024 is: “SWANs Beyond the Bedside: Supporting the transformation of care delivery”
SWANs go beyond direct patient care by advocating for individuals, helping them navigate the health care system, and ensuring their voices are heard in care planning and decision-making.
With their specialized education, SWANs offer more than just care—they uphold and share best practices that improve patient outcomes and enhance the overall efficiency of the health care system.
SWANs help support quality improvement initiatives, using their specialized training to identify gaps in care, while implementing & sharing best practices in wound, ostomy and continence care.
