Job Description
SAHN is currently seeking a contracted NSWOC (Nurse Specialized in Wound, Ostomy, and Continence) to conduct community-based, in-home assessments on a flexible, per-assignment basis. This is a dynamic opportunity for an experienced NSWOC Nurse looking to apply their expertise while maintaining independent practice and schedule flexibility.
Key Responsibilities:
Conduct comprehensive in-home assessments related to wound, ostomy, and continence care
Develop individualized care plans and recommendations based on clinical findings
Provide education and consultation to clients, families, and caregivers
Complete documentation and assessment reports in accordance with SAHN’s standards and timelines
Collaborate with referring hospitals, home care coordinators, and interdisciplinary teams as required
Uphold best practices and maintain current knowledge in NSWOC scope of care
Requirements/Qualifications
Qualifications:
Registered Nurse (RN) in good standing with the College of Nurses of Ontario (CNO)
Completion of NSWOC Certification from the Wound, Ostomy and Continence Institute (or equivalent)
Minimum 2 years of clinical experience in wound, ostomy, or continence care
Prior experience with in-home or community care preferred
Access to a reliable vehicle and ability to travel within designated regions
Strong documentation and communication skills
Professional liability insurance coverage (required for all contractors)
