    SWAN Week 2025 We are the Champions for Skin Wellness September 15-19, 2025 Celebrating SWANs across Canada SWAN Week celebrates the impact of Skin Wellness Associate Nurses (SWANs) in advancing wound, ostomy, and continence (WOC) care across Canada. This year’s theme—We are the Champions for Skin Wellness—highlights the leadership, competency, and day-to-day contributions of SWANs in every care setting. With 300+ active SWANs and SWAN students in our growing Community of Practice, this week recognizes the nurses who champion evidence-based practice, mentor colleagues, and improve patient outcomes alongside Nurses Specialized in Wound, Ostomy and Continence (NSWOCs). Who are SWANs? SWANs are LPNs, RPNs, or diploma-prepared RNs who have completed the CNA-accredited SWAN Program. They are respected nurse leaders and integral members of their organization’s wound, ostomy, and continence leadership team. SWANs: Work collaboratively with NSWOCs and interprofessional teams. Perform focused assessment and support care planning for skin, wound, ostomy, and continence needs. Coach and mentor direct care staff, building capacity across units and facilities. Apply evidence-based practice, recommend appropriate products, and help navigate referrals. Contribute to quality improvement and cost-effective care. How SWANs are “Champions for Skin Wellness” Patient-centred leadership: SWANs elevate care at the bedside while guiding teams to adopt best practices. Capacity building: They upskill nursing teams, creating confident, autonomous clinicians and consistent standards of care. Better outcomes: SWAN-led projects and case reviews help reduce wound prevalence and accelerate healing. System impact: SWANs support efficient, appropriate product selection and contribute to cost-effective care plans.     Indigenous Wound, Ostomy and Continence Health < Back Michelle Buffalo BScN, RN, NSWOC, WOCC(C) Indigenous Wound, Ostomy and Continence Health Michelle grew up in rural northern British Columbia on a large cattle farm & farming community. She moved to Alberta in 2001. She is proud to be both a Cree First Nations woman uniquely mixed with Norwegian and Latvian ancestry. She is a Samson Cree Nation band member, which is one of the four large First Nations Communities in Maskwacis, Alberta, a treaty 6 First Nations territory and also the community in which her late mother grew up. Her first role in healthcare was as a nursing attendant. Several years later she decided to follow her feather and transitioned to a Registered Nurse, graduating with a BScN from the University of Alberta in 2010. Her nursing background and experience includes working in a variety of settings such as rural nursing, emergency, geriatrics and medical/surgical areas. She has worked in case management for Maskwacis Health Services Home Care in Alberta and was also the Indigenous RN Case Manager for the Awasisak Indigenous Health Team at The Stollery Children’s Hospital in Edmonton, AB. Transitioning into the Acute RN Case Manager & Direct Care Team Lead role for Alberta Health Service's Home Care program in Wetaskiwin, Alberta. She has also worked as a relief nurse in Manitoba at a remote Indigenous nursing station for a short time. Currently, she is with the Central Zone Wound & Ostomy Consult Team for Alberta Health Service’s Ostomy & Wound Specialists Department. Working alongside other NSWOC colleagues, providing the much-needed specialized service to multiple sites in rural Alberta. Michelle has had the opportunity to work in many different settings and connect with clients from various backgrounds, journeys and cultures. This is something she is very grateful for as it brings such valuable understanding to her nursing experience and growth as a person. She graduated from the NSWOC Program in May 2021 and officially obtained her CNA Certification in WOCC(C) soon after. This incredible journey began with the starting support from her late mother and continued with the many supportive nursing mentors she has had the opportunity to cross paths with along the way. He is known as a leading Canadian nurse researcher in the areas of wound, ostomy and continence issues and has served on a number of expert panels and advisory boards to develop best practice recommendations. He was also the Early Researcher Award recipient 2014-2019 from the Ministry of Research and Innovation. This award allowed him to lead the development and evaluation of an online community designed to promote self-management of diabetes through peer-to-peer online support. He received the Journal of Wound Care’s the Best Diabetic Foot Ulcer Intervention Award in 2018 for his leadership and contribution to this area of research. In 2020, he received the Order of Merit Award from the Canadian Nurses Association. For over 20 years Dr. Woo has sustained a prolific research career, focusing on topics such as chronic disease management, knowledge translation and patient safety. He has authored over 130 peer-reviewed journal articles and 19 book chapters. He is the author of Polit & Beck Canadian Essentials of Nursing Research, a research text that has been adopted by several nursing schools in Canada. By blending his research with an active clinical practice, he has had an immediate and profound impact on the health of people living in Canada. The CNS is responsible for liaising with all program inpatient units to: proactively assist clinical teams in addressing patient care issues and clinical concerns; ensure clinical interventions are aligned with organizational policies and best practices; and, support education and academic practice, student mentorship, and research activities. A key function of this role will be to provide expertise in the care of medically complex wounds, including pressure injuries and surgical wounds, along with ostomy and continence care. Many of these patients may also have behavioural issues. More specifically, the CNS will be expected to lead Skin and Wound Rounds across the program, undertake assessments of patients with multiple medical co-morbidities, recommend appropriate surfaces and equipment, support the inter-professional care team in care planning and clinical interventions to address patient needs with respect to wound and ostomy care. As well, the CNS will be expected to provide support to patients and families by way of health-teaching, as appropriate. The CNS, Wound, Ostomy and Continence Care will further be expected to build capacity of point-of-care staff in addressing and managing both acute and chronic skin health issues. The CNS will also be expected to consult with others, such as physicians, NPs, other specialty services and supports, both internal and external to UHN, as required. The successful candidate will have many opportunities to contribute to clinical quality improvement initiatives and participate in program development in this role.
