Office Location: The work location is dependent on desk space availability and may be the work location of the successful candidate or such other location as may be mutually agreed by the Employee and the Employer (Champlain)


What will you do?

Position Summary:
The Wound Care Specialist is responsible for providing support, treatment, product recommendations and follow-up care for patients with identified problem wounds or ostomies. As a key resource, the Wound Care Specialist will interpret and apply evidence based best practice to improve the quality of patient care; and conduct outreach to clinicians and health care providers in order to facilitate adoption of services, problem solving and understanding of clinical guidelines as they pertain to identified wound care and ostomy patients.


Primary Responsibilities:
Perform Champlain-wide clinical wound and ostomy management in collaboration with members of the patient’s care team. This responsibility includes direct client consultation, direction and evaluation of individual care plans for wound and/or ostomy care

  • Act as a resource for Care Coordinators and service providers to assist in meeting patients’ needs regarding complex wound/ostomy situations where there is a heightened risk for negative outcome(s)

  • Support the development of internal policies and procedures for wound and ostomy management

  • Participate in establishing, maintaining, and monitoring standards for the wound and ostomy program. This includes external and internal committee work and active participation and contribution to quality initiatives

  • Access research pertaining to wound and ostomy products and product application related to wound care to help make sound recommendations for treatment/care

  • Action special requests for wound care supplies, request pricing and coding in the wound care supplies catalogue and approve wound care products for off formulary requests

  • Provide recommendations on alternative products within the wound care supplies catalogue

  • Participate in wound and ostomy program data collection, analysis, evaluation and recommendations

  • Monitor negative pressure wound therapy patients to ensure compliance with OH aHome Champlain guidelines

  • Answer questions or concerns pertaining to wound or ostomy care from patients, caregivers, and service providers

  • Other related duties may be assigned


Exigences/Qualifications

What must you have?

Education and Experience:

  • Registered Nurse (RN) (BScN or diploma) with a current Certificate of Registration from the Ontario College of Nurses

  • Completion of “Nurse Specialized in in Wound, Ostomy, & Continence” (NSWOC) (formerly ETN)

  • Related professional nursing experience which is normally attained over a period of five (5) years, must include experience in wound care and ostomy management

  • Membership with “Nurses Specialized in Wound, Ostomy, & Continence Canada (NSWOCC) (formerly CAET) will be considered an asset

  • Adult teaching experience and/or adult education courses is an asset

Knowledge:

  • Knowledge of wound care and ostomy management

  • Requires in depth knowledge of the variety of wound and ostomy supplies to be able to make recommendations for the supplies catalogue; provide consultation; and address any substitution requests

  • Knowledge and comfort with emerging technology and use of innovative technology (tablets, e-referral, e-health coaching)

  • Knowledge of health care system and the role of the OH aHome Champlain within the system

  • Knowledge of OH aHome Champlain operations, quality, risk, and principles

  • Knowledge of relevant health care legislation

  • Knowledge of OH aHome Champlain policies, procedures, strategic direction and goals

  • Understanding of OH aHome Champlain partners and contracted service providers in the community

Skills and Abilities:

  • Effective planning and organizing skills to manage multiple clients, provide information reports, and take corrective action as required

  • Effective interpersonal skills to work with diverse client groups with varying levels of comprehension and language capability

  • Ability to manage difficult conversations/situations with physicians, nurses and other interdisciplinary team members in a healthy and effective manner

  • Demonstrates a strong ability to work effectively in both a team environment and independently as required

  • Able to communicate effectively and concisely, both orally and in writing, including effective presentation skills

  • Effective listening and strong facilitation skills are necessary to work with patients and their families/caregivers

  • Awareness of cultural diversity

  • Time management and prioritization of tasks, as well as flexibility in adjusting to priorities in a dynamic environment

  • Diplomacy in sensitive and/or confidential issues

  • Ability to accurately complete required documentation, reports, forms, and recommendations

  • Working knowledge of computer software (email, internet) and Microsoft Office applications (Outlook, Word, Excel, Power Point, etc.)

  • A valid driver’s license and access to a vehicle is required
