Position Summary

The Nurse Practitioner (NP) in the Skin Wound and Ostomy (SWO) Program will be registered with the College of Nurses of Ontario in the Extended Class – Adult, with the advanced education and specialized skills/knowledge to provide clinical care grounded in evidence based standards for SWO patients at HHS. The NP works autonomously, and in a collaborative practice model with Infectious Disease Specialists, and in consultation/collaboration with the interdisciplinary team, interprofessional practice leaders, physicians and other key stakeholders to ensure high quality safe care according to their professional, ethical and legal standards outlined by the CNO. The components of the NP role include: Clinical Practice, Education, Research, Consultation/Collaboration Leadership and Optimizing Health Systems. Collaborating with interdisciplinary team members, and using a patient and family centered model of care, the NP is responsible to: provide direct patient care, including the assessment and treatment of patients with SWO care needs, coordination of care, and the coordination of related community and other resources for this patient population; tutor and mentor nursing and other health professional students as appropriate, and participate in continuing education activities; initiate, participate, and/or lead programs, presentations, publications, evaluations and other system changes to continue the enhancement of interdisciplinary care teams. The Nurse Practitioner will work in partnership with physicians, nursing and other members of the health care team within HHS and with community stakeholders to provide comprehensive, holistic, patient and family centered care, and to provide advanced skills, knowledge and education in health assessment, diagnoses and management, as well as health promotion and disease prevention. In addition, the Nurse Practitioner will be instrumental in working with Program Leadership and stakeholders to develop and execute strategic plans, and to advance best practices for operational excellence and sustainability in SWO care.

Exigences/Qualifications

Qualifications

  1. Current Certificate of Registration in the College of Nurses of Ontario Extended Class – Adult
    2. Completion of a Master’s Degree in Nursing
    3. Minimum of 5 years recent (within 2 years) clinical experience in an acute hospital inpatient and/or outpatient setting with thorough understanding of skin, wound and ostomy care and hospital service delivery
    4. Expert knowledge and demonstrated application of Skin, Wound and Ostomy best practices with Nurses Specializing in Wound Ostomy Continence (NSWOC) certification or equivalent program 
    5. Demonstrated leadership and critical thinking skills
    6. Demonstrated knowledge of the chronic disease management framework
    7. Demonstrated ability for independent action, initiative, problem solving, organization and prioritization
    8. Demonstrated experience in research and program development
    9. Demonstrated excellent interpersonal and communication skills, and competency to work within interprofessional teams
    10. Demonstrated evidence of commitment to continuous learning through pursuit of education endeavours.
    11. Demonstrated ability to promote professional development in nursing
    12. Eligible for Faculty appointment with McMaster University
